Claims advice for passenger carriers

In the event of an incident likely to give rise to a claim under your policy, you should advise us immediately. This may be done by telephone or e-mail to any of the following:

Relating to the aircraft and passengers:
 Certificates of registration and airworthiness
 Flight plan, list of contents of flight operations bag including relevant AERAD or other routing/approach/let down charts
 Meteorological information/briefing to flight crew
 Passenger manifest – with details of nationality, sex and age, fatalities (including documents and details of identification, death     certificates, and post mortem examinations), nature of non-fatal injuries, names and addresses of next-of-kin, hospital or funeral     arrangements.
 Copies of correspondence with passengers, next-of-kin or their legal representatives and details of social security workmen’s claims
 Load/trim balance sheets or equivalent; details of CG etc; pre-flight maintenance check or clearance certificate
 Photographs of wreckage/damage to aircraft
 Flight crew licences; route training details
 Airframe/engine logbooks
 Transcript of RT comms between the aircraft and tower/approach ATC as relevant
 Transcript from CVR
 FDR readout and analysis if applicable/available
 Copy of flight coupons or other documents issued to passengers (with complete routing); passenger seating plan
 Charter manifest, copy airway bills; details of damage to mail/freight/baggage
 Certificates of maintenance; copy last check; copy of tech log; relevant extracts from flight/operations/maintenance manuals
 Documentation relating to ownership of hull and engines, purchase, leasing, charter and lien agreement
 Notices served by local National Civil Aviation Dept relating to investigation of the accident. Please ensure that no original document is     retained by any investigation team without a clear copy being retained
 General conditions of carriage of passengers and cargo, tariffs etc. Airframe hours

 Time since last major overhaul/last inspection
 Engine type/model plus serial numbers and hours
 List of radio and navigation equipment aboard
Relating to the flight crew:
 Flight crew details required:

 Date of last proficiency check
 Date of last route check
 Date of last medical/valid to
 Total flying time last 30 days/types flown
 Length of rest period to/from
 Flight crew reported operating/route problems
 Full list of cabin crew
It will be necessary for insurers representatives to contact:
 Senior management/insurance personnel
 Senior operations/maintenance personnel
 Flight crew
 Technical personnel who arrived early at the site

Please have details of their identities/contact details and availability to hand.

Any components suspected of being the cause of the accident should be carefully preserved in co-operation with accident investigators. The components should not be released to any third party (including the manufacturer) without the consent of your insurers.
